Non-Clinical Nurse Case Manager
Location: Colchester, VT (Hybrid/Telework Eligible)
Job Summary
Provide non-clinical case management services supporting military personnel readiness. Coordinate care, track medical cases, and ensure compliance with Army medical readiness standards.
Key Responsibilities
- Manage assigned Soldier medical cases from identification to resolution
- Coordinate care across providers, facilities, and military units
- Maintain and update case files in systems (eCase, MEDCHART, MEDPROS)
- Prepare cases for Medical Evaluation Boards (MEB/PEB)
- Conduct monthly case reviews and reporting
- Support readiness events (PHA, SRP, mobilization processing)
- Educate Soldiers on medical readiness and resources
- Ensure HIPAA compliance and documentation accuracy
Required Qualifications
- Active Registered Nurse (RN) license (Vermont or eligible)
- 3–5+ years clinical or case management experience
- Strong administrative and coordination skills
- Experience with medical documentation systems preferred
Preferred
- Military / VA / DoD experience
- Case management certification
