Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and skilled nursing procedures in a home care setting
- Monitor and document patient health status, progress, and changes in condition
- Coordinate care with physicians, therapists, and other healthcare team members
- Educate patients and family members on health management, medications, and holistic wellness practices
- Ensure compliance with all state, federal, and agency regulations and standards of care
- Provide wound care, IV therapy, and other specialized nursing interventions as needed
Requirements
- Active and unrestricted Registered Nurse (RN) license in the state of New York
- Previous home care, community health, or clinical nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Reliable transportation and valid driver's license for home visits in the Medford, NY area
- CPR/BLS certification required
- Compassionate, self-motivated, and able to work independently in a home care environment
