* Collaborate with interdisciplinary healthcare teams to ensure high-quality patient care and to facilitate discharge planning
* Educate patients and their families on chronic and acute disease management, medical conditions, and post-discharge care
* Accurately document all aspects of patient care, including assessments, interventions, and outcomes, in compliance with hospital and departmental policies
