Job Description
Day Shift 7 hours/day 5 days/week
WCS Healthcare Partners is seeking a dedicated and compassionate Registered Nurse to join our hospice care team. The ideal candidate will have experience in providing high-quality end-of-life care, demonstrating strong clinical skills, and a commitment to patient and family education. This role requires a professional who can work independently, manage multiple responsibilities, and communicate effectively with interdisciplinary teams.
Schedule
- 11:00 AM – 7:00 PM
- Every Other Weekend (EOW)
- Previous ICU experience preferred
Responsibilities
- Patient Assessments
- Accurately assesses patient's appropriateness for hospice care, learning needs, and progress toward goals
- Develops the plan of care for the patient and family
- Utilization of Services
- Maintains visit productivity standards
- Demonstrates knowledge of insurance reimbursement
- Performs laptop transfer of data twice daily
- Completes admission assessment and transfers within 24 hours
- Completes re-visit documentation and transfers
- Patient & Family Education
- Uses age-appropriate teaching methods for patients and families
- Evaluates teaching plan and revises as necessary
- Documents patient education tools used
- Case Management
- Develops, implements, and evaluates plan of care based on evidence-based knowledge
- Revises care plan as indicated
- Prioritizes care needs and accommodates patient's scheduling needs
- Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines
- Shows evidence of interdisciplinary communication
- Performs HHA supervisions per standards
- Ensures appropriate utilization of services
- Follows scheduling process
- Communicates progress toward goals with IDT
- Attends and participates in IDT meetings every 2 weeks
- Evaluates the plan of care and revises as needed
- Documents case management activities
- Maintains safe environment for patients and co-workers
- Follows infection control measures and reports infections
- Demonstrates knowledge of pharmacology and safe medication management
- Routinely reconciles medications
- Maintains current medication orders
- Staff Meetings & Additional Assignments
- Attends monthly staff meetings
- Participates in PI meetings and activities/committees as requested
- Accepts other assignments as requested
- Performance Improvement Program
- Reviews health records and gathers data to support PI activities
- Attends UR meetings as requested
- Provides orientation and mentorship to new employees
- Cross-trains to learn new skills and demonstrates a willingness to teach others

Requirements
- Required Skills & Experience
- Minimum two (2) years of experience as an RN in a home hospice environment
- Experience using an EMR
- Excellent critical thinking and communication skills
- Must have a valid New Jersey driver's license
- BLS certification required
- Preferred Skills & Experience
- Three (3) or more years of RN experience
- Previous ICU experience
- Required Education
- Associate's degree (ASN)
- OR Diploma with RN License in New Jersey
- Required Certifications & Licensure
- Current New Jersey RN License
- Valid New Jersey driver's license
- BLS certification
