- Weekly Estimated Pay: $2290-$2660

- Location: Cordova, Tennessee

- Duration: 13 weeks
- Schedule: Monday-Friday, 8:00 AM-4:30 PM
- Shift Duration: 8 Hour Days
- Shifts Per Week: 5
- Hours Per Week: 40
- Coverage Area: Shelby, Fayette, and Tipton counties
- On-Call/Weekend Requirement: None
- Mileage Reimbursement: Reimbursed between visits
- Hospice nursing experience required
- Experience with HCHB required
- Current active RN license required
- Strong case management and patient coordination skills
- Ability to work independently in a home-based care environment
- Flexible, easy-going, and professional demeanor
- Provide skilled nursing care to patients in the homecare setting, including hospice-related support as needed
- Manage patient cases and coordinate care plans across the assigned coverage area
- Complete admissions and ongoing nursing assessments
- Prepare for and participate in interdisciplinary team meetings
- Document care accurately and efficiently in HCHB
- Support patients and families with compassionate education and communication
- Maintain productivity expectations of approximately 6 visits per day and 30 points per week
