Touching Angels Healthcare, a Residential Service Agency licensed by the Maryland Department of Health, is hiring a per diem Registered Nurse Case Manager to perform in-home client assessments across Maryland. You will assess clients in their homes, maintain care plans, supervise direct care staff, and ensure quality and safety standards are met, with full clinical independence in the field and administrative support from our office team.
This role fits an experienced RN who wants schedule control, per-visit compensation, and genuinely independent field work. It does not involve OASIS assessments, Medicare or Medicaid billing platforms, or office-based case management.
Duties and Responsibilities
Client Assessment and Reassessment (40%)
- Perform initial and periodic in-home assessments and update care plans on the required cycle
- Schedule assessments directly with clients and complete them within established timeframes
- Assess home safety, cleanliness, and infection control and document recommendations
Caregiver Supervision and Delegation Oversight (20%)
- Conduct required supervisory visits and evaluate direct care staff performance with skills validation
- Delegate nursing tasks only per COMAR and remain available for clinical consultation
Care Planning and Coordination (15%)
- Involve clients and families in care planning and communicate findings to physicians timely
In-Home Visit Standards and Client Relations (15%)
- Address complaints with documented investigation and verify medications, orders, equipment, and emergency protocols
Documentation and Administrative (10%)
- Complete accurate progress notes, upload assessments to the EMR, and maintain assessment trackers
Travel Requirement
CRITICAL POSITION REQUIREMENT, READ FIRST: This is a field-based role requiring extensive daily travel. You will drive between client homes across the assigned Maryland service area, typically 30 to 45 minutes between clients, for approximately 90 percent of your working time. One recurring engagement requires an approximately 2.5-hour drive each way. A valid driver's license, reliable personal vehicle, and current auto insurance are required. Do not apply if you are seeking an office-based or hybrid nursing role.
Position Requirements
- Formal nursing education from a college, university, or hospital program (Bachelor's or Associate's degree)
- Active and unrestricted Maryland RN license (or Compact state licensure valid in Maryland), verified by the HR Manager via the MBON licensee lookup
- Minimum one (1) year of pediatric, home care, or hospital experience
- Current CPR/BLS and First Aid certification
- Current professional malpractice insurance
- Health clearance: annual physical, TB screening per COMAR .B(1)(c), and Hepatitis B vaccination or documented declination
- Valid driver's license, reliable personal vehicle, current auto insurance, and willingness to travel extensively between client locations (typically 30 to 45 minutes between clients; approximately 90 percent field time)
- Ability to type and maintain a minimum of 45 words per minute
- Successful completion of a Maryland criminal background check prior to hire
Preferred: Prior Residential Service Agency or private duty home care experience; Experience supervising or delegating to CNAs and other direct care staff; Familiarity with AxisCare or a comparable home care EMR platform; Bilingual (Spanish/English).
Physical Requirements
- Stooping, turning, bending, squatting, kneeling, and the ability to lift up to 50 pounds during client assessments
- Frequent driving between client locations across the assigned service area, including one extended engagement of approximately 2.5 hours each way
- Ability to navigate stairs and varied home environments
- Visual, auditory, and tactile acuity sufficient to perform clinical assessment functions
- Potential exposure to communicable disease, bodily fluids, and household environments; standard precautions apply
Compensation
$80.00 per completed field assessment. $30.00 if a visit is cancelled upon your arrival at the client residence. $0.70 per mile between client locations. One recurring extended engagement pays $40.00 per hour for an approximately 7-hour assessment plus mileage, occurring roughly every 120 days.
Environment
Client homes across the assigned service area with daily driving between locations. Exposure to varied household environments; standard precautions apply.
Other Duties

Benefits
