Horizons Care Center seeks a compassionate Registered Nurse to deliver high-quality, resident-centered care in our skilled nursing and rehabilitation facility. The RN will assess residents, develop and implement individualized care plans, administer medications and treatments, and document in the EHR while prioritizing safety and quality. Working within an interdisciplinary team, you’ll coordinate with therapy, physicians, and families to support rehab goals and long-term care needs.
