Overview of the Role
As a Registered Nurse, you will provide both direct and indirect nursing care to assigned patients under the guidance of a PACE Clinical Services Supervisor. This role is critical in managing intricate clinical situations using your advanced nursing knowledge and skills. You will play a key part in developing patient care plans, collaborating with interdisciplinary teams, and bringing your expertise to support patient health and well-being.
Responsibilities
- Perform Assessments: Gather and document patient data, identify needs, and prioritize care using advanced nursing observations and critical thinking.
- Plan and Coordinate Care: Develop and revise comprehensive care plans in collaboration with patients, families, and interdisciplinary teams, and ensure seamless communication of patient changes.
- Implement Care: Deliver and manage care consistent with nursing standards; educate patients and families; recognize emergencies and independently initiate interventions; document care accurately.
- Evaluate Care: Monitor and record patient progress, address any deviations in care plans effectively, and make necessary adjustments.
- Lead and Supervise: Delegate tasks, ensure cost-effective practices, participate in quality improvement, and contribute to team staffing needs and safety protocols.
- Engage with Teams: Be an active member of the Interdisciplinary Team, participate in care planning, and support collaborative patient management.
- Complete Other Duties: Engage in additional projects and tasks as needed.
Requirements
- Graduate of an accredited nursing school.
- Current RN license in good standing.
- Minimum of 3 years recent RN clinical experience.
- Preferred experience in paperless documentation and working with frail or elderly populations.
- Must have reliable transportation, valid driver's license, and mandatory auto insurance.
- Medical clearance for communicable diseases and up-to-date immunizations.
- Pass a comprehensive background check.
- Ability to work effectively in a team, communicate openly, understand geriatric patient care, and demonstrate basic computer skills.
