Home Health Care ManagementHome & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.Overview of the RoleThis role is for a Registered Nurse to work with an interdisciplinary care management team supervising the care of clients who are receiving home and community-based services.Responsibilities:To perform this job, you must be able to perform the following duties successfully:Review of Medical Records including a client's Plan of Treatment, menu of health services, medication, medical equipment and other community-based services to ensure that the right amount and type of care and products meet the client's medical needs.Communicate with providers/ patient participants or health care department or HUD housing representatives both verbally and in written format regarding clients' care needs.
Utilize electronic health record as required by departmental procedures to document all face-to-face or telephone or zoom communication with the client, their caregivers, medical providers and social service agencies.Work with an interdisciplinary team caring for clients.Institute tracking of quality indicators and reports to improve patient outcomes.Coordinate with Case Management team members (RN and Social Work Case Managers) via zoom or in-person meetings, or joint visits to clients as needed.Conduct monthly visits with participants via phone, teams, or in personEssential Requirements:Current California State license as a Registered NurseValid California Driver's LicenseEligible to be insured under our liability policy with a clean DMV reportYour own vehicle and proof of current auto insuranceProficiency using software programs, and familiar with Microsoft office product including Word and ExcelAbility to work independently and with others at all levels within the organization and collaborate effectively.Above-average interpersonal, problem-solving, written and oral communication skillsCPR certificate is required within 30 days of employmentPreferred Skills, Knowledge and CertificationsProfessional behavior when on dutyDischarge planning, home health or hospice experienceAt least one year of work experience as an RNInquisitive nature and analytical skillsService Region/ Location:This role is located in Sacramento serving Sacramento County and surrounding counties.This is not a remote role and requires you to work in our Sacramento office.Schedule :Full time, 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch. No weekend work.Currently 20% of the time is related to direct home visits, 80% of the time are telehealth video visits and or calls and documentation in the office.Travel :This role involves some travel to and from clients' homes.Physical Requirements:Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically includes the ability to lift to 35 pounds without assistance.PPE is provided to all employeesCompensation:Pay range is $43 to $53 per hour.
