Key responsibilities

- Identifies and manages members according to established criteria, facilitating necessary authorizations and referrals
- Monitors and evaluates the effectiveness of care management plans, making modifications as needed
Required qualifications
- Minimum 5 years of clinical case management experience or equivalent education and experience
- Associates or Bachelor's degree in Nursing; BSN preferred
- Current, unrestricted RN license in the applicable state
- Ability to communicate effectively and collaborate with diverse groups
- Strong problem-solving, organizational, and time management skills
