Role:
RN Case Manager, responsible for patient-centered care coordination to improve health outcomes.
Key Responsibilities:
- Assess, plan, implement, monitor, and evaluate care options to optimize patient health.
- Develop individualized care plans, integrating clinical guidelines and promoting quality and efficiency.
- Collaborate with patients, families, and healthcare teams for coordinated services.
- Monitor progress through diagnostics, utilization patterns, and metrics; adjust plans as needed.
- Manage utilization and practice metrics to enhance clinical, quality, and fiscal outcomes.
- Maintain thorough documentation of case management activities.
- Assist in designing and evaluating advanced patient-centered care models.
Qualifications & Skills:
- At least 2 years of RN work experience; case management experience preferred.
- Certified Case Manager/ACMA certification preferred.
- BSN preferred; knowledge of healthcare laws, regulations, insurance policies, and healthcare IT systems is helpful.
- Ability to work independently and collaboratively within a multidisciplinary team.
