Key responsibilities Identifies and acts upon impending changes in member conditions to ensure appropriate care Analyzes issues impacting care plans and initiates corrective actions as necessary Maintains a budgeted caseload while ensuring member goals are achieved within specified timeframes Required qualifications Current registered nurse licensure in the state of Maryland or LCSW-C Bachelor's degree in nursing or related field, or MSW in social work preferred Minimum five (5) years of clinical practice experience Minimum three (3) years of clinical practice in a specific specialty area Certification in case management (CCM) preferred
