Responsibilities
- Care coordination for hospitalized patients.
- High‑risk screenings and assessments.
- Coordinating post‑acute care services with proper documentation.
- Collaborating with multidisciplinary team members.
Experience Requirements
- At least 1 year of case management experience.
Education & Credential Requirements
- Associate’s Degree in Nursing.
- Basic Life Support (BLS).
- Driver’s license (DMV).
- Active Registered Nurse (RN) license.
