Role Overview
As an RN Case Manager, you coordinate and deliver high-quality, patient-centered home health care. Your responsibilities include:
- Providing direct care to patients with acute and chronic conditions in their homes
- Documenting assessments and validating patient needs
- Promoting patient autonomy and helping them adapt to changing health situations
- Applying strong critical-thinking skills to ensure safety and comfort
- Preventing adverse events and supporting positive health outcomes

Qualifications
