Position Overview

We are seeking a dedicated professional to join our team for a full‑time role with a schedule of 5/8s, including every other weekend. This position requires a California RN License and BLS certification, along with a minimum of 5 years of RN Case Management experience.

Key Responsibilities

  • Care Coordination and Transitions Planning: Collaborate with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely patient progression and transition to appropriate care levels, aiming to prevent unnecessary admissions or readmissions.
  • Patient Assessment: Review initial physician admission care plans, gather additional information, and conduct screenings for ancillary supportive services.
  • Utilization Management: Ensure patient records meet level‑of‑care requirements, expedite transition planning, and collaborate with financial counselors and attending physicians.
  • Care Transition Formulation: Develop transition plans in collaboration with patients, families, and healthcare teams, addressing complex clinical and social situations efficiently.
  • Documentation and Compliance: Maintain current knowledge of regulatory requirements, consistently document in EHR, and ensure compliance with state and federal regulations.
  • Operational Contribution: Identify and implement new systems and practices to improve patient/provider experiences and resource utilization.
  • Communication and Collaboration: Foster positive relationships with healthcare teams and community agencies, utilizing effective communication skills to promote customer service.
