To support optimal wellness for high-risk members, the full-time Remote Registered Nurse Case Manager - NICU, who must be bilingual in Haitian Creole, will assess member needs, develop individualized care plans, and coordinate services across the healthcare continuum. Key responsibilities Conduct comprehensive assessments of members' physical, psychological, and social needs to create tailored care plans Monitor and adjust care plans to ensure effective service provision and member progress towards health goals Interface with healthcare providers to facilitate care coordination and provide education to members regarding their treatment and services Required qualifications Bilingual in Haitian Creole Registered Nurse (RN) with a current, unrestricted compact license Three or more years of clinical experience in a Level III or above NICU CCM certification required within specified timeframe Experience in a Case Management role preferred
