Base pay range
$55.00/hr - $60.00/hr
Duties
- Perform care coordination and discharge planning for patients.
- Utilize InterQual Criteria for admission criteria in the Emergency Department.
- Participate in the development of treatment plans and family meetings.
- Work with interdisciplinary support including Social Services and Transportation.
- Arrange Durable Medical Equipment (DME), Skilled Nursing Facility (SNF), and ambulance transport independently.
- Manage patient diagnoses, including drug/alcohol abuse treatment, homelessness, mental health crises, and more.
- Document patient care using computerized systems.
Skills
- Proficient in Microsoft applications.
- Experience with HMOs, IPAs, and related healthcare management organizations.
- Competence in arranging patient transport and equipment independently.
Experience
- Minimum of 2 years of acute care hospital case management experience, specifically in care coordination and discharge planning.
- Experience with Epic charting system is required.
- Travel experience is required, with first‑time travelers accepted.
- Must have a CA state license or be licensed upon the start date.
- Experience with CMS and DRG is preferred.
