Overview
HealthPlus Staffing is seeking a Registered Nurse Case Manager to provide care and support to terminally ill patients. This will be a full-time position primarily serving Merced and South Stanislaus Counties.
Responsibilities
- Provide compassionate hospice care to terminally ill patients, focusing on symptom management and emotional support.
- Coordinate and deliver patient care as a nurse case manager in the Merced and South Stanislaus Counties region.
- Apply nursing care processes and adhere to medication guidelines and side-effects management.
- Maintain high ethical standards and safety precautions while delivering care and support to patients and families.
Qualifications
- Minimum one year nursing experience; hospice or home health experience preferred.
- Knowledge of nursing care processes and medication guidelines/side-effects.
- Strong commitment to safety, ethics, and patient confidentiality.
- Proficient in reading and writing English.
- Excellent communication skills and a compassionate nature.
- Cool-tempered, patient, with good emotional stability and stamina.
- California Registered Nurse license required.
- CPR certification current.
- Must have reliable transportation and auto insurance.
- Hospice experience preferred.
