Salary range: $88,263.96 - $124,602.44 per year

How you'll make an impact in this role

• Coordinate the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient and family preferences and values.
• Monitor patient care through assessments, evaluations, and record reviews, serving as a resource for proper medical documentation, service appropriateness, and post-discharge treatment options.
• Advocate for necessary resources, eliminate care barriers, and maintain ongoing dialogue with supervisors and healthcare providers for effective plan implementation.
• Oversee patient progress and care quality while facilitating clear communication among all members of the healthcare team.

What minimum qualifications you'll need

Licensure / Certification / Registration:

• Required Credential(s): Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or jobtransfer date.
• Preferred Credential(s): BLS Provider. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time ofhire.

• Previous in patient discharge experience highly desired
