Registered Nurse - Case Manager

Join our team as an RN-Case Manager, providing in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses caused by exposure to toxic substances.

Key Responsibilities Include

• Conduct weekly in-home nursing visits, including physical assessments, medication management, home safety checks, and overseeing caregivers.
• Develop and evaluate care plans, coordinate with clients and healthcare teams, and ensure quality of care.
• Serve as a clinical resource on-site, support staffing and scheduling, and adhere to agency policies and scope of practice.
• Participate in staff training, review clinical documentation, and contribute to performance improvement initiatives.
• Assist with client and physician engagement and perform supervisory visits as needed.

Qualifications Include

• Graduate of an approved nursing program with an unrestricted RN license.
• BSN preferred, with at least two years of nursing experience, including one in home care.
• Current CPR certification and supervisory or case management experience preferred.
