Schedule
Shift: Monday to Friday, 5 x 8‑hour days from 08:30 to 17:00. No on‑call requirement, but the option to pick up weekend shifts is available if needed.
Responsibilities
- Float throughout patient tower across MedSurg units as necessary.
- Coordinate care and manage discharge planning.
- Assess patient needs and order Durable Medical Equipment (DME).
Requirements
- MD License.
- BLS Certification.
- 3 years of experience as a Case Manager in an outpatient clinic or acute hospital setting.
- Experience with Care Coordination and Discharge Planning.
- Familiarity with CMS and HIPAA regulations.
- Medical Inpatient Experience is a must.
- Flu vaccination is required for all employees.
