Daily Responsibilities:
- Telephonic position to ensure medical necessity and appropriate level of care. Develop, assess and adjust as necessary the care plan and promote desired outcome.
Hours for this Position:
- Monday through Friday 8am-5pm Full Time with medical benefits offered

Qualifications
- 2+ years of clinical nursing
- 2+ years of home health Case Management experience
