Key Responsibilities
* Manage and oversee patient care within hospice settings, ensuring high‑quality, patient‑centered services
* Develop, implement, and evaluate individualized care plans
* Coordinate with interdisciplinary healthcare teams, including physicians, social workers, and support staff
* Provide leadership, mentorship, and guidance to nursing staff
* Maintain accurate, timely clinical documentation
* Communicate effectively with patients, families, and caregivers
* Participate in on‑call rotation (one evening weekly + one weekend monthly)
* Travel locally to patient homes and facilities across multiple counties
Required Qualifications
* Education: High School Diploma or GED
* Experience:
* 1+ years of Hospice experience
* 1+ years of Management or leadership experience
* 1+ years of experience in ANY of the following:
* Home Health Care
* Epic Home Health
* Home Health
* Licensure:
* Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
* Certifications (ALL required):
* American Heart Association Basic Life Support (AHA BLS)
* American Heart Association Advanced Cardiac Life Support (AHA ACLS)
* Other Requirements:
* Reliable transportation and ability to travel locally
* Strong time‑management and self‑organization skills
* Ability to work independently in field‑based environments
