Location: Tulsa, OK

Facility: Ascension St. John's Medical Center

Department: Case Management

Schedule: Full-time, Monday-Friday, 8am-4:30p

How you'll make an impact in this role

  • Improve hospital efficiency and finances by reducing stays, streamlining patient flow, and optimizing resource use, which boosts revenue and minimizes waste.
  • Enhance care transitions, lower readmissions, and connect patients with community resources, leading to better satisfaction and addressing social needs.
  • Support clinical teams by bridging gaps, advocating for patients, and ensuring comprehensive, timely care, resulting in higher quality, lower costs, and better patient outcomes.

What minimum qualifications you'll need

Licensure / Certification / Registration:

  • Registered Nurse obtained prior to hire date or job transfer date required.
  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What additional requirements you'll need

  • 3+ years of RN experience
  • Case Management experience preferred
