To support the health plan's medical management team, the full-time Registered Nurse Care Manager will coordinate care for SNP members, establish care plans, and collaborate with interdisciplinary teams, all while working remotely. Key responsibilities Conduct intake assessments and develop individualized care plans for members Act as a liaison between members, caregivers, and healthcare providers while managing transitions of care Utilize NCQA standards to oversee care management processes and ensure quality care delivery Required qualifications Current Registered Nurse license issued by the state or multi-state RN license through the eNLC Three years of healthcare clinical experience One year of experience in Care Management, Case Management, or Population Health Experience managing Medicare, Medicaid, or SNP populations is preferred Bachelor's Degree in Nursing, Associate of Science in Nursing Degree, or Diploma is preferred
