Registered Nurse Care Manager

To support chronic disease management initiatives, the full-time Registered Nurse Care Manager will conduct comprehensive telephonic assessments, collaborate with interdisciplinary teams, and manage patient referrals in a remote work environment. Key responsibilities Provide chronic disease management support for value-based care contracts, including patient education and care planning Conduct comprehensive telephonic assessments to identify medical, psychosocial, behavioral, and environmental needs affecting patient health outcomes Collaborate with an interdisciplinary care team to coordinate patient-centered care and manage registry-based referrals Required qualifications Active Registered Nurse license obtained prior to hire date Diploma from an accredited school/college of nursing 3-5 years of experience in care management, case management, social services, community health, or clinical practice Proficiency in electronic health records and care management software Certification in Case Management (CCM, ACM, or similar) preferred
