Managing medically complex patients, the full-time Registered Nurse Care Manager - Compact License will provide telehealth care management, prepare Annual Wellness Visit charts, and support preventive care initiatives in a fully remote setting with evening and overnight shifts. Key responsibilities Prepare comprehensive Annual Wellness Visit charts and communicate findings to providers Conduct nursing assessments and develop individualized care plans for chronic care management Identify and close care gaps while coordinating care across multiple services and providers Required qualifications Active, unrestricted Registered Nurse (RN) license in a compact state Minimum 3 years of clinical nursing experience, including telehealth or virtual care settings Experience in Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) Proficiency with electronic health record systems and telehealth platforms Ability to work independently in a remote environment with a dedicated workspace
