Responsibilities
- Collaborate with clinic team members to provide evidence-based patient-centered care
- Help patients stay up to date on care plans and goals
- Focus on high-risk medically complex patients, patients on disease registries, and care transitions
- Act as a liaison with patient’s family members and other agencies
- Facilitate improved communication for comprehensive whole person care
- Stay up to date on standards, assisting with programs and processes
Requirements
- Associate Degree in Nursing from a CCNE or ACEN accredited nursing program
- Current Ohio Registered Nurse license
- CPR/BLS certified
- Knowledge of triage procedures
- Clinic, physician office and/or urgent care experience
- Ability to maintain quality, safety, and/or infection control standards
- Knowledge of appropriate procedures and standards for administration of medications and patient care aids
- Ability to educate and clearly communicate with patients and/or families regarding disease nature and proper care and treatment
- High level of interpersonal skills to handle sensitive and confidential situations
- Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions
