A well-established skilled nursing facility in Far Rockaway, Queens is hiring an experienced MDS Coordinator. This is a full-time MDS Coordinator role paying up to $130,000 per year, and it requires a Registered Nurse (RN) with prior MDS Coordinator experience in a skilled nursing setting — this is not a training position.

What You'll Do
- Complete and coordinate MDS (Minimum Data Set) assessments accurately and on schedule
- Manage the RAI (Resident Assessment Instrument) process and lead care plan meetings with the interdisciplinary team
- Ensure documentation supports accurate PDPM (Patient-Driven Payment Model) reimbursement
- Maintain compliance with federal and New York State assessment requirements
- Work with nursing and therapy teams to keep assessments aligned with residents' actual clinical status
Qualifications
- Current Registered Nurse (RN) license in New York State — required
- Prior experience as an MDS (Minimum Data Set) Coordinator — required
- Experience in a skilled nursing facility (SNF) / long-term care (LTC) setting — required
