Registered Nurse – Acute Care – Care Coordinator FT Days

Seeking a Registered Nurse (RN) Care Coordinator to join a hospital-based team, responsible for assessing patient needs, coordinating care across the continuum, and facilitating smooth transitions from hospital to next care levels.

Key duties include conducting psychosocial and functional assessments, developing individualized discharge plans, collaborating with physicians and interdisciplinary teams, and managing barriers to timely discharge. The role involves facilitating patient and family education, participating in rounds, and coordinating post-acute referrals to ensure efficient care progression.

Required qualifications encompass an associate’s degree in nursing , a valid Georgia RN license, and a minimum of one year of acute care nursing experience . Certifications ​(​the ​i  ​af? maybe missing cut?) BLS are mandatory upon hire.
