Position: Registered Nurse (RN) Home Care Assessment & Field Supervisor

Key Responsibilities:
Initial Nursing Assessments & Re-evaluations: Conduct comprehensive initial clinical evaluations and mandatory ongoing reassessments for new and existing clients in their homes across New Jersey.

Plans of Care Development: Create, review, and adjust individualized Plans of Care (POC) based on client needs, physician orders, and program requirements (Medicaid, ABD, DDD, and Private Pay).

Caregiver Supervision & Field Audits: Perform mandatory 60-day/90-day supervisory home visits to observe Home Health Aides (HHAs) and Direct Support Professionals (DSPs), ensuring adherence to the care plan and maintaining compliance standards.

Clinical Guidance & Staff Training: Provide orientation, skill evaluations, and clinical instruction to home health aides and caregivers on specialized client care protocols.

Care Coordination & Compliance: Collaborate with case managers, family members, physicians, and administrative staff to ensure high-quality care delivery, accurate documentation, and complete compliance with NJ state home care regulations and HHAeXchange updates.
