Role Description the Registered Hospice Nurse Case Manager is a full-time, on-site role located in Watauga, TX. This role is responsible for managing a caseload of hospice patients, performing comprehensive assessments, and developing individualized care plans in collaboration with the interdisciplinary team. Day-to-day responsibilities include providing direct nursing care, monitoring symptoms, coordinating medications and treatments, and educating patients and families about disease processes, comfort measures, and available support services. The Nurse Case Manager facilitates communication among physicians, hospice team members, and caregivers, ensuring timely updates, smooth transitions of care, and adherence to regulatory standards. The role also includes documenting all care and interventions accurately, participating in team meetings, and engaging in ongoing quality improvement and best-practice initiatives.
Qualifications
- Candidates should possess strong Nursing and Medicine skills, with the ability to provide expert hospice and palliative care.
- Candidates should possess Case Management and Discharge Planning skills to coordinate care plans and transitions effectively.
- Candidates should possess Utilization Management skills to ensure appropriate, efficient use of hospice services and resources.
- Active Registered Nurse (RN) license in the state of Texas, in good standing.
- Prior hospice, palliative care, or home health experience is strongly preferred.
- Excellent communication, documentation, and interpersonal skills, with the ability to support patients and families during end-of-life care.
- Ability to work collaboratively in an interdisciplinary team, prioritize multiple patients, and maintain professional boundaries and ethical standards.
- Current CPR certification and valid driver’s license; reliable transportation for patient visits as needed.
