Serving as the main point of contact for the agency's EMR/Referral portals, the full-time Referral Navigator will manage prompt responses to referral requests while demonstrating professionalism and excellent customer service in a remote setting. Key responsibilities: Manage EMR/Referral portals in accordance with Referral Acceptance Guidelines Provide timely responses to referral requests and communicate with referral sources Conduct pre-checks on patient insurances and escalate referrals requiring clinical approval Required qualifications: Associate's degree preferred At least one year of experience in home health and customer service Proficiency in managing EMR portals and Microsoft Office suite Familiarity with medical terminology is preferred Strong data entry skills and ability to work efficiently in a fast-paced environment
