In this role, you will coordinate recovery plans, connect clients to community resources, and support housing, employment, and treatment goals. You will maintain regular contact through home and community visits, monitor progress, and advocate for client needs.
Working within a trauma-informed, equity-focused team, you’ll collaborate with clinicians, case managers, and partner agencies, maintain accurate documentation, and uphold agency and regulatory standards.
