Position Summary

Position Summary
The Licensed Clinical Social Worker serves patients of the Network. He/she is a member of the patient-centered health care team at the assigned location(s), and coordinates care activities with other Providers.
Essential Duties and Responsibilities:
To perform this job successfully, an individual must be able to perform each essential duty:
- May be assigned to offer "traditional" mental health services, which includes assessment, diagnosis, establishment of treatment goals and care plans, provision of mental health care and coordination
- Attentive to general medical concerns and treatments, and supports such interventions
- May be assigned to an "integrative" role, in which case he/she serves as a Behavioral Health Consultant (BHC) to a defined set of primary care providers
- The BHC assists primary care providers through brief targeted assessment, addressing mental health concerns with brief interventions, supporting health related behavior change, and triaging need for formal mental health care
- Provides assessment & diagnosis
- Orchestrates care planning and care
- Serves as a liaison to active coordination with other health and mental health providers
- Carefully and thoroughly documents clinical services; ensures codding accuracy
- Consults with primary care providers; collaborates on best course of treatment
- Maintains a productive and efficient schedule
- Continually focus on patient concerns, complaints and patient satisfaction

The requirements listed below are representative of the knowledge, skill and ability to perform the essential functions:
- Must be certified to practice in New York State as a Licensed Clinical Social Worker, with a preference for NYS Department of Insurance "rating" to practice independently (the R rating).
- Must meet all network credentialing requirements as set by the medical staff by-laws
- Demonstrate the appropriate skills, knowledge and abilities to successfully fulfill the requirements of the position
- Demonstrated written, verbal, organizational, problem-solving and decision-making skills
- Knowledge of community services, health plan rules, support agencies and governmental regulations
- Electronic Medical Record system experience; athena is a plus
- Proficient in Microsoft Office
The pay rate for this position is $78,000/annually.
