To support a growing healthcare initiative, the part-time Puerto Rico Licensed Clinical Pharmacist will conduct Comprehensive Medication Reviews (CMRs), establish protocols for patient care, and collaborate with a multidisciplinary team to enhance medication adherence through remote engagement. Key responsibilities Build trust-based relationships with patients through telephonic and video engagement Deliver Medication Therapy Management (MTM) services, including CMRs and medication reconciliation Proactively address social determinants of health affecting patient adherence and connect them to appropriate resources Required qualifications Must reside in Puerto Rico Active Pharmacist License with at least 3 years of pharmacy practice Medically fluent in English and Spanish Demonstrated experience in patient relationship-building roles such as health coaching or chronic disease management Comfort with motivational interviewing and behavioral health screening techniques
