Connecting with patients through daily video check-ins, the part-time Puerto Rico Licensed Clinical Nurse will provide encouragement, education, and accountability to support medication adherence and improve health outcomes in a remote setting. Key Responsibilities: Conduct patient outreach and engagement through video messaging, consultations, and phone calls to educate and enroll patients in the adherence program Utilize video-based communication to deliver personalized education, coaching, and adherence support while monitoring medication-taking behavior Provide compassionate coaching and case management to motivate patients towards medication adherence and lifestyle changes Required Qualifications: Must be a legal resident of Puerto Rico Registered Nurse (RN) license in good standing with at least 5 years of nursing experience 5 years of related experience in patient engagement, health education, or care coordination Fluency in both English and Spanish, with proficiency in reading, writing, and speaking Strong background in chronic disease management and medication adherence
