Description
  • Conduct patient assessments in both inpatient and outpatient settings, tailoring individualized treatment plans to their unique needs
  • Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
  • Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimum outcomes
  • Guide patients and their families through therapeutic goals, discharge planning, and designing home exercise routines
  • Maintain detailed records of evaluations, treatments, and patient progress as per facility and regulatory standards
  • Engage in multidisciplinary conferences and care team meetings to ensure seamless care delivery
Education
  • Doctorate in Physical Therapy (DPT) or equivalent credential from a recognized program
Certifications
  • State licensure as a Physical Therapist
  • CPR certification preferred
  • Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
  • Prior experience in a similar role is strongly preferred
