We are seeking a compassionate and skilled Physical Therapist to join our clinical team in Aurora, IL. Primary Clinical Responsibilities

• Assessment & Planning: Conduct comprehensive patient evaluations across inpatient and outpatient settings to develop individualized, evidence-based treatment plans.
• Therapeutic Intervention: Implement targeted physical therapy protocols to enhance patient mobility, strength, neuromuscular balance, and overall functional capacity.
• Patient Education: Instruct patients and caregivers on therapeutic goals, discharge safety, and home exercise programs (HEP) to ensure continuity of care.
• Clinical Documentation: Maintain meticulous records of evaluations, progress notes, and treatment outcomes in strict accordance with regulatory and facility standards.

Interdisciplinary Collaboration

• Care Coordination: Participate in multidisciplinary care team meetings and interdisciplinary conferences to ensure holistic patient management.
• Communication: Partner effectively with physicians, nursing staff, case managers, and rehabilitation specialists to ensure seamless, coordinated patient outcomes.
• Advocacy: Act as a clinical guide throughout the recovery continuum, adapting strategies to meet the unique physiological and psychosocial needs of each patient.
