PsychiatricMental Health Nurse Practitioner

A behavioral health organization in Queens, New York is seeking a Psychiatric-Mental Health Nurse Practitioner to provide community-based psychiatric assessment, medication management, and collaborative care for adults with complex behavioral health needs.
About the Opportunity:

• Shift: Day shift
• Schedule: Monday to Friday
• Hours: 9am to 5pm
• Setting: Hybrid (3-4 days field-based, 1-2 days remote)

• Conduct comprehensive psychiatric evaluations and diagnostic assessments
• Diagnose mental health disorders and develop individualized treatment plans
• Prescribe, monitor, and adjust psychiatric medications in alignment with evidence-based practices and ACT standards
• Provide education to clients and families regarding diagnoses, treatment options, medications, and potential side effects
• Collaborate with multidisciplinary ACT team members and support crisis intervention, care coordination, documentation, and treatment planning

• Minimum of 2 years of experience in psychiatric or behavioral health settings
• Master's Degree in Nursing
• Active New York State Psychiatric Mental Health Nurse Practitioner (PMHNP) license
• Board Certification as a Psychiatric Mental Health Nurse Practitioner
• Experience working with individuals with serious and persistent mental illness (SPMI)
• Strong knowledge of psychiatric disorders, psychopharmacology, and integrated care practices
• Strong clinical judgment and assessment skills
• Excellent communication and interpersonal skills
• Ability to work independently and collaboratively within a multidisciplinary team
• Comfort working in mobile, community-based treatment settings

• Bilingual (English and Spanish)
• Previous ACT team experience
• Experience with community-based behavioral health programs
• Experience treating individuals with co-occurring substance use disorders
• Familiarity with electronic medical records, preferably AccuMED
