Job Description
We are currently seeking experienced Psychiatric Mental Health Nurse Practitioners (PMHNP-BC) to join our collaborative team providing psychiatric care to patients in partnered nursing facilities across Virginia's Eastern Shore. This is a facility-based role centered on our nursing home care model. Care is delivered primarily in person, on-site at the locations we assign, with some visits provided via telehealth. We are looking to fill a full, steady schedule, so we are prioritizing providers with substantial availability ideally someone we can keep busy full time.
What We're Looking For
Active PMHNP-BC license in Virginia
Knowledge of Gradual Dose Reduction (GDR) is highly preferred including its application in long-term care settings and psychotropic medication management consistent with CMS and facility regulatory requirements. Training on GDR is available for strong candidates who are eager to learn.
Prior Medicare credentialing/enrollment we are prioritizing providers who have previously been credentialed with Medicare.
Ability and willingness to conduct in-person visits at skilled nursing and residential care facilities throughout Virginia's Eastern Shore care is delivered primarily in person, with some visits via telehealth.
Substantial weekly availability we are ideally seeking a provider we can fill with a full-time schedule, and we will prioritize candidates who can commit significant, consistent hours.
Experience working with a diverse patient population, including Medicare and Medicaid patients in long-term care.
Ability to operate independently as a solo provider or within a small collaborative team.
