Job Description

Community Bridges, Inc. Psychiatric Mental Health Nurse Practitioner

As a psychiatric practitioner at CBI, you will provide patient‑centered, evidence‑based care for adults 18 and older, all genders, all socio‑economic backgrounds, complex medical history, and co‑occurring psychiatric disorders. You will serve as a patient advocate in all health‑related matters and coordinate care with specialists and community resources.

Responsibilities

  • Complete medical practitioner admission orders and coordinate CON with the CPEC Medical Director within 24 hours of admission.
  • Complete a full psychiatric evaluation (HPI) and document the evaluation for each admitted patient within 24 hours.
  • Review and modify treatment plans to meet psychiatric, medical detoxification, and physical health needs.
  • Participate in daily rounds with RN and Transition Coordinator, including discharge planning.
  • Complete inpatient rounds between 0800 and 1600 and document rounds in the progress note.
  • Monitor treatment of physical health and coordinate with PCP and other prescribers.
  • Initiate or recommend consults and coordination of care, ensuring timely collateral information.
  • Provide documentation and treatment for patients under petition or court‑ordered treatment, including a Special Treatment Plan.
  • Prepare and sign full discharge orders and follow‑up recommendations within 24 hours of discharge.
  • Prescribe appropriate pharmacotherapy for psychiatric disorders, coordinated care for physical health, substance abuse treatment, and pain management in accordance with accepted standards.
  • Maintain at least 80% e‑prescribing accuracy, targeting 100%.
  • Evidently document evaluation of each patient for addiction medicine protocol, Suboxone taper, ongoing mental health/psychiatric care, and physical health prior to discharge.
  • Complete orientation, TB skin test, suboxone waiver, and CPR/First Aid certification as required.
  • Maintain documentation compliance of at least 85% and patient satisfaction of at least 85%.
  • Maintain a minimum 75% follow‑up rate and complete informed consents within 24 hours of patient encounter.
  • Use e‑prescribing for all patient prescriptions and review the Controlled Substance Prescription Monitoring Program.
  • Participate in quality management initiatives, utilization review activities, and utilization management activities as needed.
  • Adhere to scheduling protocols, provide coverage, and give at least 60 days’ notice for time off.
  • Respond promptly to emergencies and coordinate coverage as directed by the Director of Medical Practice and Medical Director.
  • Other duties as assigned.

Documentation

Medical Professionals must complete documentation within 24 hours of each visit. If documentation is incomplete, notify the Director of Medical Practice.

Training and Professional Development

Medical Professionals may provide or receive training for new clinical staff. Attend educational meetings and quarterly practitioner meetings; a 24‑hour notice is required if unable to attend.

Integration

Practice limitations should be acknowledged, and consultation with other healthcare professionals sought when necessary to provide optimal patient care. Utilize CBI’s integrated model of behavioral, physical, and psychiatric services.
