Provider Services Nurse Practitioner (NP) (Servicing Bitterroot Valley)
JOB SUMMARY
The Provider Services Nurse Practitioner (NP) at Big Sky Home Health & Hospice supports the organization’s mission of providing life‑changing service by helping patients receive primary care services as they navigate a complex medical system. This role focuses on clinical service delivery, provider support, care coordination, and operational excellence across Big Sky's care continuum. The Provider Services NP collaborates closely with the Executive Director, clinical leadership, and interdisciplinary teams to ensure high‑quality, cost‑effective, timely, and compliant care delivery. All services are provided within the NP’s scope of practice and aligned with organizational standards, regulatory requirements, and best practices.
DUTIES & RESPONSIBILITIES
Under the supervision of the Executive Director, the Provider Services NP:
- Provides clinical support and services that promote safe, effective, and efficient healthcare delivery across Big Sky's service lines as a primary care physician.
- Supports operational clinical workflows, provider services initiatives, and care coordination efforts to ensure continuity and quality of care.
- Delivers clinical services via in‑person visits, telephone, or telehealth encounters across multiple care settings, including residential, facility‑based, and home environments.
- Serves as a clinical resource to interdisciplinary teams, supporting evidence‑based practice, quality improvement initiatives, and best‑practice standards.
- Performs comprehensive patient evaluations—including history taking, assessment, diagnosis, treatment planning, prescribing medications, patient education, and evaluation of care outcomes.
- Collaborates with physicians, advanced practice providers, nursing staff, care coordinators, and operational leaders to optimize patient outcomes and service delivery.
- Ensures compliance with state‑specific standards of practice for Nurse Practitioners and all applicable federal, state, and local regulations.
- Documents clinical findings, assessments, and care plans accurately, thoroughly, and timely in the electronic medical record.
- Maintains strict patient confidentiality and safeguards protected health information at all times.
- Participates in provider services initiatives, clinical protocol development, and quality assurance activities as assigned.
- Maintains a safe working environment for self, patients, families, and staff by following infection control standards and utilizing appropriate personal protective equipment (PPE).
- Engages collaboratively with internal teams and external partners including healthcare providers, community resources, patients, and family members.
- Represents Big Sky Home Health & Hospice professionally at all times, fostering a cooperative and respectful work environment through tact, diplomacy, and a positive attitude.

PRODUCTIVITY & PERFORMANCE EXPECTATIONS
- Meet established productivity, quality, and operational performance standards as defined by Provider Services leadership and the Executive Director.
- Complete all clinical documentation accurately and within 7 days of each patient encounter or service provided.
- Participate in on‑call or clinical support rotation as required to meet organizational needs.
JOB REQUIREMENTS
- Master’s degree from an accredited Nurse Practitioner program with current and unrestricted state licensure, prescriptive authority, and credentialing eligibility.
- Ability to successfully pass background check and drug screening.
- Maintain a valid driver’s license, access to reliable transportation, and willingness to travel as needed.
- Fluency in reading, writing, and speaking English.
- Strong verbal and written communication skills with the ability to communicate effectively with providers, operational leaders, patients, families, and community partners.
- Working knowledge of applicable federal and state regulations, including HIPAA, OSHA, and healthcare compliance standards.
CLINICAL COMPETENCIES
- Comprehensive Assessment
- Perform a thorough assessment considering physical, cognitive, and psychosocial aspects.
- Assess and diagnose acute and chronic illnesses and identify risk factors.
- Evaluate functional status including ADLs and IADLs.
- Chronic Disease Management
- Manage common chronic conditions in adult and elderly populations (e.g., diabetes, hypertension, osteoarthritis).
- Develop and implement personalized care plans for chronic disease management.
- Coordinate patient care with MA / support staff and delegate tasks as appropriate.
- Medication Management
- Knowledge of geriatric pharmacology.
- Medication reconciliation and management, considering potential interactions and side effects.
- Palliative and End‑of‑Life Care
- Provide palliative care and manage end‑of‑life issues.
- Facilitate discussions about advance care planning.
- Fall Risk Assessment and Prevention
- Assess and address fall risk factors.
- Implement strategies to prevent falls in the elderly.
- Cognitive Function Assessment
- Conduct cognitive assessments (e.g., Mini‑Mental State Examination).
- Identify and manage cognitive impairments.
- Communication and Patient Education
- Strong communication skills to interact with patients, families, and healthcare professionals.
- Explain complex medical concepts in clear and understandable manner.
- Cultural Competence
- Provide culturally competent care and recognize importance of family dynamics in geriatric care.
- Patient and Family Education
- Educate patients and families about health conditions, medications, and self‑care.
- Promote health literacy and empower patients to actively participate in their care.
- Home Care Competencies
- Assess patient home environment for safety and make recommendations for improvements.
- Use telehealth technologies for remote patient monitoring and virtual consultations.
- Know community resources and services available to support patients at home or in assisted living.
- Administrative Competencies
- Time management and scheduling for house calls.
- Accurate and timely documentation, coding, and billing of services based on established guidelines.
- Collaboration with other healthcare professionals, including primary care physicians, specialists, and home health agencies.
- Understanding of legal and ethical issues related to geriatric care, including informed consent and patient confidentiality.
- Commitment to ongoing learning and staying current with advancements in care.
WORKING ENVIRONMENT
- Exposure to blood & bodily fluids, latex, medicinal preparations, tuberculosis (TB), hazardous drugs, high‑risk medications, and infectious and contagious diseases.
- Exposure to varying weather conditions, unpredictable risks in patient environments, odors, and chemicals.
PHYSICAL REQUIREMENTS
- Essential senses: hearing, talking, vision, depth perception, touch, and smell.
- Ability to maintain stable posture and mobility with hands free for routine and emergent patient care.
- Cardiovascular and physical endurance; ability to work extended hours as necessary.
- Weight lifting up to 50 lbs maximum, 25 lbs frequent, 10 lbs occasional.
- Activity requirements may include: sitting, standing, walking, bending, kneeling, squatting, reaching above and below shoulder level, pushing/pulling objects, repetitive hand movements.
