Job Summary
The Provider Performance Advisor serves as a trusted advisor and consultant to healthcare provider practices, helping improve operational performance, quality outcomes, patient care, and value-based care performance. The role analyzes population health data, performance metrics, and practice workflows to identify improvement opportunities, drive practice transformation, support sustainable behavior change, and deliver measurable results.
Key Responsibilities
- Support provider practices in implementing value-based care programs and performance improvement strategies.
- Improve access to care, preventive care, chronic condition management, and overall quality outcomes.
- Support efforts to reduce avoidable hospital and emergency department admissions and readmissions.
- Improve appropriate medical coding, documentation, and billing practices.
- Align provider practices with quality and performance goals.
- Establish and monitor weekly, monthly, and annual operational performance goals.
- Evaluate practice behavior changes and quality performance against established targets.
- Analyze population health data and present performance results to practice and organizational leadership.
- Identify barriers to performance and recommend practical improvement strategies.
- Drive sustainable operational and behavioral change through practice transformation.
- Monitor, document, and report practice progress and recommend next actions.
- Support effective use of population health management tools and digital data systems.
- Facilitate implementation of digital data retrieval and integration processes.
- Train and support provider practices on performance improvement and value-based care strategies.
- Build strong relationships with physicians, practice leadership, and clinical staff.
- Collaborate with internal teams and maintain feedback loops regarding provider performance and improvement activities.
Required Qualifications
- Bachelor's degree in Business, Public Health, Healthcare, or a related field, or equivalent experience.
- 3+ years of healthcare industry experience, preferably in provider practice management, quality, medical assisting, coding, or revenue cycle.
- At least 1 year of experience with Medicaid/Medicare value-based care and population health management.
- Experience with HEDIS and STARs quality measures.
- Knowledge of medical coding, billing, and revenue cycle processes.
- Experience with EHR/EMR systems.
- Demonstrated ability to drive practice transformation and performance improvement.
- Strong provider engagement, consulting, communication, and relationship-building skills.
- Ability to analyze performance data and translate findings into actionable improvement strategies.
- Practice management experience is preferred.
- Healthcare quality certification such as CPHQ is preferred.
- Medical Assistant credentials or experience are preferred.
- Experience with EMR template development is preferred.
