Geriatrics & Extended Care ServiceThe position is located in the Geriatrics & Extended Care Service, Home Based Primary Care (HBPC). HBPC is an interdisciplinary team with a physician, nurse practitioners, registered nurses, social workers, pharmacists, psychologist, dieticians, and rehab therapists who provide primary medical and mental health services in the home eligible homebound veterans.
