As a Program Specialist, you will play a critical role in shaping each child's care journey - coordinating services, building individualized plans, and ensuring every child has the opportunity to reach their fullest potential.This is more than a case management role. Join our team today!Friendly and supportive staff, Strong leadershipWhat You'll Do - As a Program Specialist, you will serve as a key coordinator and advocate for assigned individuals:Develop, manage, and update Individual Support Plans (ISP/IHP) and 6-month reviewsCoordinate interdisciplinary input to ensure comprehensive, person-centered care planningLead and facilitate required meetings in compliance with PA 3800/6400 and DHS regulationsTrain and support staff on individualized plans, behavioral supports, and safety protocolsMonitor and document monthly and quarterly progress toward outcomesEnsure all required services are delivered and aligned with each child's goalsMaintain consistent communication with families, schools, and external providersParticipate in interdisciplinary, QA, and safety meetingsSupport participation in community-based activities and life experiencesQualificationsBachelor's degree in Psychology, Sociology, Human Services, or related field + 2 years IDD experience OR Associate degree (60+ credits) in related field + 4 years IDD experienceExperience working directly with individuals with Intellectual and Developmental Disabilities (IDD)Strong organizational, documentation, and coordination skillsExcellent communication and relationship-building abilityComfort working across multiple disciplines and developmental levelsProficiency with Microsoft Office and general computer systemsValid driver's license; ability to attend off-site meetingsCPR/First Aid Certification (or willingness to obtain)Must be 21 years of age or olderEOE StatementWE ARE AN EQUAL OPPORTUNITY EMPLOYER.
