Job TitleProgram SpecialistDutiesHelpDuties include but, are not limited to:Maintains full responsibility for Primary Care Management Module (PCMM) as PCMM coordinator in the primary care setting.Provides program support to the Ambulatory Care Service clinic leaderships and staff conference calls related to access, wait times, and clinic profile management.Responsible for supporting the clinic profile and PCMM based training for new Medical Clinic Directors, including how to use the LEAF portal to submit profile changes and providing new employee orientation training for providers on clinic grids, submitting time off or clinic block request and entering time for timecards.Identifies problems and makes recommendations in scheduling, quota allocation of providers and supervisory staffing and recommends alternative courses of action to ensure efficiency, economy, and balance in the operation of the Primary Care clinics.Performs clinic and data management to include analyzing, researching, computing, and providing statistical data, reports and special studies required of Primary Care Clinics and Ambulatory Care Service.Maintains and updates provider records with information required by Joint Commission on approximately 70+ providers.Sets up clinic profiles for Ambulatory Care Service primary care clinics at the Durham VA Healthcare System including Hillandale 1 CBOC, Raleigh-Clayton CBOCs, Garner VA Clinic, PRIME education clinic and Comp & Pension clinic.Serves as a staff analyst, providing direct recommendations and analysis to Primary Care leadership regarding access and capacity.Maintains the PACT model staffing report and presents data monthly to facility leadership. Acts as a resource and deep subject matter expert to clinical staff in trouble shooting of Vista and PCMM software applications associated with the Primary Care Clinic treatment area.Develops and maintains panel dashboard data, assisting management in balancing workload management. Primary Care Management Module (PCMM) Coordinator reviews PACT model data to ensure compliance with VA policy and directives, as well as identifying new program polices and procedure regarding PACT management.Assist in the annual inventory of property for the service; also assist with the preparation of requests for supplies, equipment, work orders, etc.
