What You’ll Do
- Manage the day‑to‑day activities of the assigned program.
- Monitor services for your assigned client caseload.
- Ensure compliance with program and licensure standards.
- Support and collaborate with office staff.
- Report concerns to the Lead Program Director.
- Travel for client visits and community activities.
What You Bring
- College degree in Social Work or a related field (required).
- 2+ years of experience providing services to individuals with disabilities (required).
- Case management experience (preferred).
- Home health experience (preferred).
- Familiarity with CLASS, CBA, DBMD, PHC, or DADS programs (a plus).
- Strong organizational, communication, and leadership skills.
