Position Title

PRN RN Case Manager – Home Health

Employment Type

Part‑Time, Full‑Time, PRN, Contractor, or Hybrid. The position offers long‑term growth opportunities to a full‑time RNCM role within 6–12 months based on performance and agency expansion.

Service Area

Indianapolis, IN and surrounding 2‑hour radius. Typical travel distances are 10–60 minutes with occasional visits up to 2 hours. Travel is required to all client sites throughout Indiana.

Schedule

Primarily weekdays 8:00 a.m.–5:00 p.m. with early evening and rare weekend shifts available. No overnight or on‑call shifts are required. Schedule is flexible and can be customized to fit other personal or employment commitments, provided weekday daytime availability is maintained.

Responsibilities

• Conduct client assessments, plan, initiate and coordinate appropriate care actions in home‑care settings.
• Manage and supervise a multidisciplinary team of RN, LPN, Home Health Aides, and Personal Care Aides.
• Document, communicate, and coordinate patient status changes to the care team.
• Perform case management duties including admissions, start‑of‑care (SOC), discharge planning, recertifications, transitional care, and resumptions of care (ROC).
• Provide skilled nursing visits, wound care, PICC/PORT/IV care, lab draws, and other assessment services.
• Lead new‑hire orientations, in‑services, and supervisory reviews.
• Support training and educational initiatives that influence care practices and outcomes.
• Travel to multiple counties as required and report directly to the Indiana Regional Program Administrator (local) and Director of Clinical & Administrative Operations (hybrid, Ohio).
• Assist in building relationships with client insurance payors and case management entities such as AAA, DoDD County Board, VA, and others.

Qualifications

• Graduate of an accredited School of Nursing.
• Current Registered Nurse license in Indiana and at least one year of nursing experience.
• Minimum one year of paid experience in a home health or acute care setting.
• Minimum one year of experience in case management.
• Experience with OASIS and non‑OASIS assessments.
• Experience in wound care, lab draws, PICC/PORT/IV care, and transitional care.
• Leadership or supervisory experience preferred (minimum one year).
• Basic CPR and First Aid certifications; additional qualifications may be obtained prior to hire.
• Strong written and verbal communication skills; confidentiality and HIPAA compliance.
• Reliable transportation and a valid driver’s license.
• Desirable traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful, and positive.

Compensation

Hourly wage: $36–$40 per hour, paid weekly on Fridays; full‑time RNCMs may be eligible for salary (paid twice a month).
