Responsibilities
- Evaluate physician medication orders for appropriateness and potential interactions; recommend therapy adjustments
- Maintain patient database with medical history, lab data, and medication records
- Order and evaluate lab tests for high-risk conditions
- Collaborate with healthcare team members
- Document all communications in medical records
- Ensure safe procurement, storage, and disposal of medications
- Maintain documentation in compliance with regulations
- Report medication errors to the Pharmacy Manager
- Assist staff with medication procedures
- Perform other duties as assigned
Qualifications
- Bachelor’s Degree in Pharmacy; Pharm D preferred
- One year experience in long-term care
- Licenses/Certifications as required by regulations
- Attention to detail, goal-oriented, team player, good communication skills
